Rolling Hills Eye Care PLLC offers a wide range of eye care services, including but not limited to:

The services we provide are subject to availability and may vary depending on your insurance coverage or medical condition.
3. Patient Responsibilities
As a patient, you agree to:

- Follow our prescribed treatments, procedures, and advice.
- Inform us of any changes in your health status or vision concerns.
- Notify us of any cancellations or appointment changes at least 24 hours in advance.

5. Appointments and Cancellations
- Scheduling Appointments: You can schedule appointments online through our website online scheduler, via phone or text, or via email at team@rollinghillseye.care
- Late Arrivals and Missed Appointments: If you are more than 15 minutes late, your appointment may need to be rescheduled.
- Cancellations: We ask for at least 24 notice for cancellations..
6. Payment and Billing
- Payment Methods: We accept a variety of payment methods, including insurance, credit cards, and cash.
- Insurance: We accept or are actively being credentialed most major health insurance plans, and we will work with you to verify your benefits before your appointment. We are not “in-network” with any vision plans at this time.
- Out-of-Pocket Payments: You are responsible for any co-pays, deductibles, or out-of-pocket costs that are not covered by your insurance.
- Billing Disputes: If you have questions about your bill, please contact us on our main line.
7. Conduct and Behavior
Patients are expected to treat our staff and other patients with respect and courtesy. Disruptive or abusive behavior will not be tolerated and may result in the termination of services.
8. Medical Records
We maintain your medical records according to applicable laws. You have the right to request access to your records, request corrections, and request copies for a fee as outlined in our medical records policy.
9. Limitation of Liability
Rolling Hills Eye Care is committed to providing high-quality care, but we do not guarantee specific outcomes from medical treatments. Our liability for any claims, losses, or damages arising from the use of our services is limited to the amount paid for the specific service provided.
10. Telemedicine Disclaimer
Telemedicine consultations are intended for non-emergency situations only. We will make every effort to provide appropriate care, but telemedicine consultations have limitations compared to in-person visits. You agree to provide accurate information during the consultation and understand the limitations of virtual healthcare.
11. Termination of Services
You or Rolling Hills Eye Care PLLC may terminate the patient-provider relationship at any time, for any reason, subject to applicable laws. If we choose to terminate services, we will provide you with appropriate notice and assist in transitioning your care if necessary.
13. Changes to Terms
We reserve the right to modify or update these Terms of Use at any time. Any changes will be posted on our website with an updated effective date. By continuing to use our services after any changes, you accept the updated Terms of Use.
14. Governing Law
These Terms of Use are governed by and construed in accordance with the laws of Blanco County. Any disputes will be resolved in the courts of Blanco County.
